The standard for breast reconstruction using implants has typically involved the placement of a tissue expander which allows the muscles to be stretched over three or more months, creating a space for the final implant.

With Real Breast Reconstruction, an implant is placed immediately at the time of mastectomy. A muscle substitute called Alloderm is used to construct an Internal Bra support for the implant, removing the need to stretch the chest muscle. This method works best with size C or smaller breasts. With this method, the breast can be recreated in a lower, more natural position than typical reconstruction, and looks good even immediately after the surgery. Immediate implant reconstruction is especially good for patients undergoing bilateral mastectomies, as it yields a very symmetrical cosmetic result. The final scar is minimal, or it may be completely absent.
